Our hospice care team is composed of compassionate individuals who are committed to caring for others. Our team includes:
Medical Directors: Physicians with a specialty in pain and symptom management who collaborate with the hospice team and the patient’s primary care physician to oversee the plan of care.
Registered Nurses: Specialists in hospice care and experts in pain and symptom management who are available around the clock.
Certified Home Health Aides: Specially trained and available to assist patients with personal care and daily activities such as bathing, shaving, etc.
Medical Social Workers: Patient and family advocates who assist with the utilization of community resources, insurance and facility issues, financial matters, funeral arrangements, and placement decisions.
Chaplains: Spiritual-care counselors who provide non-denominational faith support and bereavement support to patients, families, and staff members.
We also work with hospice care volunteers who help make a difference in the lives of the people we serve.
