Our medical social workers offer social, environmental, and emotional counseling for patients with end-of-life and/or financial problems, as well as coping mechanism and long-term care needs. We can be there for you in the difficult times of your life, overcoming your psychosocial challenges.
Our medical social services include, but are not limited to:
- Advocacy and community resources
- Community services and resources, such as adult day care or senior centers, or programs for rent exemption or reduced utility costs.
- Entitlement programs such as Lifeline, Dial-a-Ride, and Meals-on-Wheels.
- Services provided by agencies affiliated with specific illnesses, such as the American Cancer Society or the Alzheimer’s Foundation.
- Public benefit programs including Social Security, Medicare, AHCCCS, ALTCS, VA benefits, and food stamps.
- Assessment for home safety
- Supportive counseling
- Family counseling
- Individual counseling
- Assistance with burial arrangements
- Long-term care planning
- Caregiver support
- Discharge planning
